Watch for the OrthoFi email
A welcome email arrives with a secure link to your forms. It’s the same system we use for your account and your payments, so you only sign up once.

Fill them in whenever suits
Ten minutes at your kitchen table. If the patient is under 18, a parent or guardian completes and signs.

Privacy
Your information is protected and kept private.
Your forms go into your patient record and nowhere else. We don’t share them, and we don’t use them for marketing.
